• Valid Identification
Make sure to bring a valid, unexpired form of identification, such as a driver's license, passport, or government-issued ID. We are required to verify your identity before proceeding with any services.
*Expired identification information WILL NOT be accepted.
• Request Forms
For both live scan fingerprinting and notary services, it's important to have the correct request forms filled out and ready to go. Whether it's for a background check, licensing, legal document, or any other official purpose, having the necessary forms prepared will expedite the process.
• Payment Fees
Payment is required prior to processing live scan or notary service (applicant background clearance fee if applicable, rolling service fee and concierge mobile fee).

Where Do I Get the Live Scan Forms?
Applicants must get instructions and the forms required for fingerprinting services from the agency requiring their criminal history background check. Please complete the Applicant information prior to your appointment. Some agencies provide forms on their website for download (e.g. DMV).
We DO NOT provide agency request forms at your appointment.
